Central Valley Health

All 7th grade students will need a meningococcal conjugate vaccine (MCV4) and Tdap before attending this 2020-2021 school year.  HPV vaccine is highly recommended at this age but not required.  All 11th grade students will need a second dose of meningococcal conjugate vaccine (MCV4).  Also, your child must be at least 16 years of age at the time of receiving the vaccination in order for it to be valid.

Central Valley Health District is planning to hold a drive thru vaccination clinic on July 15 and August 5 from 9 am to 3 pm at the Jamestown Civic Center (front door at the ramp).    To schedule an approximate time to arrive please call CVHD at 252-8130.    Paperwork will need to be completed.  You can print and complete the immunization form off of centralvalleyhealth.org or request a form to be mailed.  Completed paperwork can be dropped off at CVHD or it can be sent with the student at the time of the vaccination clinic.

Central Valley is hoping this will be a convenient way for your student to receive the required vaccinations.  You do not need to come with your child during the vaccination clinic – HOWEVER you will need to sign the required consent form which can be done in advance.

Please call 252-8130 for more information.
