Sports Physicals completed for the 2019-2020 school year will be accepted during the 2020-2021 school year upon submission of a parent and athlete signed waiver (previously known as Form B). This waiver is available in the documents section on our school website. Students who did not have a Sports Physical for the 2019-2020 school year will need to complete one prior to participation in any NDHSAA sanctioned sport.  Please call the school if you have any questions.
